Registered Nurse – Modality Nurse

This is a Part-time position in Scarborough, ON posted April 28, 2021.

Your Opportunity: Under the direction of the Unit Manager, the Alberta Kidney Care-North (AKC-N) Modality Registered Nurse is responsible for increasing patient knowledge and informed decision-making in respect to the optimal treatment for kidney failure, given patients’ circumstances, abilities, and/or physiological health factors.

The role will have education, clinical, quality, and research components guided by the standardized provincial modality education guidelines.

Where appropriate and consistent with the patients’ wishes, education will promote living donor transplantation, home dialysis, and conservative kidney management.

The role involves close collaboration with key operational and physician stakeholders in Chronic Kidney Disease (CKD) , HemoDialysis, Home Dialysis, and Transplant.

The RN is responsible for collaboratively developing and executing initiatives for individuals and groups of AKC-N patients to assure modality education.

The role will focus on providing education and follow up for unplanned dialysis stats, failing transplants, and CKD patients.

The RN will act as a resource for the multidisciplinary team regarding Renal Replacement Therapy.

Will be required to travel all AKC-N sites.

Description: As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.

In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.

You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Classification: Registered Nurse (2022.75) Union: United Nurses of Alberta Department: UAH Renal Insufficiency Clinic Primary Location: U of A Hospital Multi-Site: At or Out of (UNA only) FTE: 0.40 Posting End Date: 25-JAN-2021 Employee Class: Regular Part Time Date Available: 04-FEB-2021 Hours per Shift: 7.75 Length of Shift in weeks: 2 Shifts per cycle: 4 Shift Pattern: Days, Evenings, Weekends Days Off: As Per Rotation Minimum Salary: $36.86 Maximum Salary: $48.37 Vehicle Requirement: Driver’s License, Vehicle Required Required Qualifications: Completion of an accredited nursing education program.

Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA).

Current Basic Cardiac Life Support
– Health Care Provider (BCLS-HCP).

Additional Required Qualifications: BScN required.

Three to Five years recent Nephrology Nursing experience within the past year required.

Experience in adult education, including group presentations required.

Ability to make sound clinical assessments and ability to communicate findings in a concise and accurate manner.

Demonstrated excellent assessment skills including the ability to assess the patients suitability for home dialysis or transplant.

Must be able to lift 40lbs and assist with patient transfer.

High proficiency with Microsoft Word, Excel, and PowerPoint required.

Possession of a valid driver’s license and access to a motor vehicle in good repair for work related travel is required.

Preferred Qualifications: Canadian Nephrology certification.

Experience in home dialysis either peritoneal dialysis or home hemodialysis Experience in CKD clinic Experience with Connect Care and Nephrology Information System (NIS).

Previous experience with chronic disease management and case management.
