Reporting to the Clinical Manager of the Nurse-Led Outreach Team (NLOT), the Registered Nurse (RN) role in the NLOT program is primarily responsible for providing clinical care to support residents living in Long-Term Care (LTC) homes within a specific sub-regional boundary. The RN works in collaboration and consultation with the NLOT NP, physicians, coordinators, and other members of the inter-professional healthcare team to prevent avoidable Emergency Department visits and hospitalizations and to support transitions to and from the hospital when needed. The RN will link the hospital and the LTC homes to ensure the most appropriate and efficient clinical care provision. The RN will also support NLOT program development and direction that is agile and iterative, reflective of the dynamic nature of resident care needs and evolving Ontario Health and Ministry of Long-Term Care priorities.


• Practice in accordance with the Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, The Long-Term Care Homes Act, the Practice Standards set by the College of Nurses of Ontario and standards
• Assist in avoiding hospital admissions for current LTC residents who have new or increasingly complex medical or specialized equipment needs which cannot currently be accommodated in their LTC home
• Support placement of eligible high-needs designated Alternate Level of Care (ALC) patients from hospitals into LTC
• Enable the admission of community members with specialized needs to LTC homes
• Assist in reducing non-urgent transfers to the region′s Emergency Departments
• Provide capacity building for LTC staff to develop and enhance competency for acute treatment protocols
• Provide assessment, intervention, monitoring and follow-up for acute and chronic illnesses
• Provide leadership and promote excellence in patient-centred nursing care through expert practice, education, consultation and research
• Actively participate in a Regional NLOT Community of Practice
• Provide input and support in the development and implementation of integrated hospital-community-LTCH clinical pathways
• Develop linkages with external partners as appropriate to ensure continuity of care
• Provide direct patient care to LTC residents
• Utilize the Clinical Information System
• Utilize the protective equipment provided and identifies and report hazards


• Bachelor′s Degree in Nursing
•Current Registered Nurse registration with the College of Nurses of Ontario
• Current Basic Cardiac Life Support (BCLS) certification required
• Acute medicine experience within the last two years
• Excellent interpersonal and communication skills, both written and verbal
• Demonstrates excellent problem-solving, decision making and critical-thinking skills
• Demonstrates excellent organizational, time-management and team-building skills
• Demonstrates initiative and the ability to function both independently and as an inter-
professional team member
• Flexible and adaptable regarding assignment locations
• Proven strong patient assessment and observation skills with the ability to exercise
sound judgment
• Demonstrates a commitment to customer service when dealing with patients,
colleagues and volunteers and other members of the community and public
• Demonstrates values consistent with SHN′s Code of Conduct and is committed to
fostering a healthy and positive work environment in accordance with Health & Safety
policy and legislation.
• Demonstrate an understanding of and commitment to SHN’s comprehensive safety
programs and practices, including staff safety, patient safety, and environmental
safety and participates in regular in-service education in this area.
• Demonstrates excellent work performance and attendance record
• Engages in ethical conduct and strives to understand and appreciate the diversity of
our patient/staff population and community
• Valid Ontario Driver’s license and access to a car required
• Clinical expertise including but not limited to: IV initiation, wound care, feeding tube
care, catheter care
• Apply clinical knowledge and leadership skills to collect and interpret information, make appropriate clinical decisions, and conduct therapeutic nursing interventions.
• Their care is characterized by up-to-date, ethical, and resource-efficient clinical practice and effective communication with patients, families, and clinical teams.
• Must have a wide range of nursing skills related to general medicine nursing.
• Must have demonstrated leadership skills, excellent critical thinking and the ability to adapt to unpredictable situations.
• Motivated self-starter with the ability to work independently and collaboratively as part of a team
• This position will work in off-site community settings providing clinical care to support residents living in Long-Term Care (LTC) homes within a specific sub-regional boundary in Scarborough

Physical Demands:

• Assists with lifting, transferring, ambulation of the patient etc.
